At Family Veterinary Mobile Clinic , we’re committed to protecting the privacy and security of your information.  Your trust is imperative to our success and we are pretty darned protective about what information we share, to whom, why, and when.

We collect information that is necessary for us to properly care for your pets and to provide the service you request.   The medical record is a legal document that details the reason you asked for our services, our findings, the recommendations we make (including those that you decline), test results, and any conversation written or oral that we have regarding your pet’s care.  We are legally required to maintain a medical record for each pet for 3 years after the last service.  Our medical records are locked down at midnight.  After that, we can not change the record except to add to it.  The amendments are time and date stamped.  This is a legal requirement to prevent insurance fraud and inaccurate records.  

Your pet’s medical record is always available to you through our pet portal.  Please do not share your pet portal password with anyone you don’t want to have access to your pet’s information.  The pet portal is part of ezyvet, our practice information management system.  Their privacy policy is here.  

Data Collected

Data About You and Your Pet

In order to provide services to you and your pet, we need your name, address, phone number, text number, and email address.  This information is collected in our New Client Form.  On that form, you have the option to opt out of text.  You can also opt out of text at any time by replying STOP to any text we sent.  

We communicate primarily by email to send your pet’s medical records which include examination findings, test results, and recommendations.  We also send appointment reminders and confirmations via email.  For that reason, if you opt out of email, we can no longer provide services to you.

Once every quarter, we send a newsletter that has practice information and updates.  We may occasionally send a newsletter at other times if we are going to be closed, have added a new service, etc.  

We require a complete medical history from all other veterinarians your pet has ever seen before we provide care.  This information is vital for our veterinarians to provide the high standard of care we are known for.  Your previous veterinarian may not respond to our requests for records; this is because they value your privacy.  You may need to contact your previous veterinarians to release the medical records to us.  

In order to provide services for your pets, we share your name, address, phone number, email address, pet’s name, age, species, breed, sex and some medical information with the following companies:

  • Covetrus, our online pharmacy, in order to provide prescriptions for your pet.
  • Any person or entity you listed on your New Client Form 
  • Any time we are legally compelled to provide data
  • Any other online pharmacy from which you requested medications

Financial Data

Financial data is NOT part of a medical record and is not disclosed to anyone for any reason unless we are legally compelled.  We do not keep credit cards “on file” or store them in our practice information management system, on our phones, computers, tablets, or any other device.  Transactions with us are made through gateways and processors that have the most secure encryption and malware protection available.  

Payment Processor Privacy Policies
  • Clover is used for in-person veterinary care purchases via credit card and ApplePay.
  • CareCredit may be used for in-person veterinary care purchases.
  • WooCommerce is used for online store purchases.
  • You may use Affirm for online store payment plans.
  • We use Stripe for payment plans and for processing online payments for pet sitting and other services.
  • Covetrus is our online pharmacy.
